To register at our parish, you may print the English version or Spanish version of the registration form and fill it out. Once complete, please email the form to records or bring it by the office.

You may also register by completing and submitting the form online.  Your information will be loaded to our parish database and printed to be stored in our secure records storage area.  If you would prefer to print the form and deliver it directly to the office, you are absolutely welcome to do that.  Welcome to Sacred Heart!
